Care management includes steps to assure a client receives the needed services in a supportive, thorough and cost effective process. Care management can include medical, psychosocial, functional, financial, and other services which respond to a client's needs.
Our care managers are problem solvers, advocates, service coordinators, and counselors. With an established professional network, we have an intimate knowledge of resources and facilities in the community. Who Needs Care Management?
If any of these circumstances describes your situation, our care managers can help create solutions:
- An older adult has a new diagnosis (dementia, stroke, cancer) and needs help accessing resources and assembling a care team.
- Family members live at a distance, are unfamiliar with local resources or cannot monitor or manage care for an older adult on their own.
