We are looking for a talented and passionate Activity Specialist to join our team of professionals and assist with implementation of our unique program. This is a wonderful opportunity to employ your creative and programming skills, working dynamically and independently within a strong team environment. The Activity Specialist works one-on-one with clients in their homes or places of residence, and leads the creation and implementation of additional projects geared toward improving client quality of life. Our Activity Specialists create customized activities and outings, working with the support of the entire team to enrich clients’ quality of life.

Essential Duties Responsibilities of the Activity Specialist include, but are not limited to, the following:

  • Lead creation and implementation of a variety of therapeutic activities aimed at improving client quality of life.
  • Plan and implement individualized activity sessions for a diverse range of clients, conducting sessions one-on-one with clients in their places of residence and group programs in facilities.
  • Leading art based group activities in assisted living facilities.
  • Conduct regular visits to home care clients to provide materials and support for QOL activities.
  • Comply with Sage Eldercare’s Policies and Procedures and all applicable laws and regulations.


  • Possesses a strong entrepreneurial sense: proficient at driving program and project development from inspiration to completion.
  • Exemplary writing skills: ability to write reports of professional tone and quality after each activity session.
  • Shows exceptional creativity and competence developing and implementing individualized activity plans and leading group activities.
  • Excellent people skills: able to connect with a wide range of individuals, engage them, and put them at ease.
  • Strong leadership skills: adept at providing ongoing quality of life activities trainings and support to care providers.
  • Outstanding time management: ability to be consistently on time for each client visit and meeting throughout the day, send reports promptly, and manage time spent on projects effectively.
  • Patience and empathy for the ill, injured, frail and those with disabilities. Experience and familiarity with cognitive changes and dementia.
  • Compliance with legal documentation for work.
  • A minimum of three references from existing or prior employers.
  • Must have a current and valid driver’s license and be able to safely operate a vehicle in compliance with all applicable laws and regulations with or without reasonable accommodations.
  • Master’s degree, or relevant experience, preferred.
  • At least 3 to 5 years’ experience working with seniors.

Working Conditions

  • Office, in clients’ homes and assisted living facility (client residence) environments.
  • Involves driving to various clients homes throughout area; may involve driving client in your vehicle.
  • Reports directly to Lead Activity Specialist.
