Once you have downloaded the Checklist, you can now record the needs of your parent in each area, including: Health, Memory, Physical Appearance, Functional, Behavior, and Diet. It’s important to complete this form monthly so you have a new baseline for comparison to spot any changes – which could be a red flag that needs to be addressed. You are now able to prioritize the needs and create a plan to address each of these needs. One note: you may choose to complete this form electronically to send (via email or print) to family and friends to provide the latest status of your parent or older adult.
